The unit was named a Best Nursing Home for 2022-2023 by U.S. News & World Report, placing the unit among the top 5 percent of skilled nursing facilities in Illinois. The unit was ranked in the top 38 for all nursing facilities in Illinois out of 703 total nursing homes, hospital officials said.

Jacksonville Memorial Hospital’s 15-bed transitional care unit is located within the hospital but is considered a short-term care facility for patients transitioning from the hospital to home or from one care setting to another. In addition to the Best Nursing Home status, the unit also received a high performing rating for short-term rehabilitation.

“Our transitional care unit provides a crucial service for local residents who need additional care before returning home,” said Dr. Scott Boston, Jacksonville Memorial Hospital president and CEO. “Not only does it benefit our patients, it also provides peace of mind for their loved ones.”

The nonprofit hospital’s transitional care unit earned Best Nursing Homes status by achieving a rating of “High Performing,” the highest possible rating for a short-term rehabilitation facility, Boston said. U.S. News gives the designation of Best Nursing Home only to those with excellent performance in quality measures and those that satisfy U.S. News’ assessment of the appropriate use of key services.

“Our staff works closely with the JMH inpatient nursing teams to coordinate patient care and share best practices,” said Jessica Ford, nurse manager of the transitional care unit. “This enables us to provide care that meets the specific needs of our patients. We’re proud of this national recognition, which shows that our work is making a difference for the people we serve.”
